Ask The Expert: Is there a Client Assessment completed prior to the start of service and if so, what does it involve?

Most providers will typically send a representative out to a client’s home to conduct an initial assessment. This usually involves getting some feedback on the client’s condition and what their needs may be. Based on this feedback, a Plan of Care is usually completed. This Plan of Care is essential in outlining the duties of the caregiver upon his/her arrival to the client’s home. The Plan of Care can be revised throughout the duration of care through follow-up visits or “re-assessments”.
