• Hepatitis B vaccination • MMR (proof of 2 doses of measles and 1 dose of mumps if born on or after January 1, 1957 and 1 dose of rubella) • Tetanus-Diphtheria toxoid (TD) within the last ten years, Varicella Vaccine (2 doses) (or a signed and dated letter stating that you have had chicken pox) • Submit a complete immunization record • Submit a background check • Social Security Card (required for state testing)
